The small differences are you need to always use the cheat death passive and play safer (easier difficulties).In softcore, there's no penalty for dying. You die, you just go back. Hardcore, one death and you're done, which adds a fair bit if excitement and fear. It's a rush when you almost die and don't. You also build your character completely different; more defensively.
